To secure your appointment, a deposit of $99 is required at the time of scheduling. If you need to cancel or reschedule, please provide us with at least 48 hours' notice. Please note that missed appointments or cancellations without adequate notice will result in the forfeiture of your deposit.
*If x-rays indicate the need for scaling and root planing (D4341,D4342 SRP), you will not receive a prophylxaxis cleaning at this appointment; however, you will receive a fine scale prophylaxis following your final SRP treatment.
